I give the Town of Ajax permission to photograph/film either myself or the child(ren) named below and to publish said photographs/videos in Town of Ajax communications channels, including marketing and promotional materials (digital & print) and the official Town of Ajax website and social media channels.
I understand that the photographs/videos are the exclusive property of the Town of Ajax and may be reproduced by the Town and anyone it has authorized, without compensation or payment to the individual(s) concerned or any other person.
I have read the above authorization and release, prior to its signature by me. I understand the authorization and I confirm that I have had the opportunity to have it explained to me and that any questions I may have with respect to the authorization have been answered to my satisfaction.
Personal information on this form is collected pursuant to the Municipal Act, 2001, s.10, and will be used for the purpose of giving the Town of Ajax the authority to use, publish or print photographs/videos taken at Town events or programs.
