By checking this box you are acknowledging on behalf of the Entertainer that you have read, fully understand, and agree to the Entertainer Rules and Regulations as well as the Insurance and Liability requirements. I further acknowledge that failure by the Entertainer to comply with the intent of the Festival or Event, the Entertainer Rules and Regulations and the Insurance and Liability requirements outlined may result in the Business or Organization being denied participation in the festival or event.

If accepted to participate in Town of Ajax Festivals and Events, I confirm that my Company has no outstanding violations and/or contraventions with the Ontario Fire Code, municipal by-laws or the Ontario Building Code.
