Recreation & Culture Volunteer Confidentiality Agreement

As part of the Town of Ajax's Volunteer Program, you will be required to ensure that your performance meets the Town's expectations. These expectations are listed below for your review. If at any time throughout your volunteer placement, your performance does not meet the Town's expectations, you may be subject to dismissal. Therefore, it is very important you understand what is expected of you during your volunteer placement with the Town. If you do not understand the expectations listed below, or have questions regarding such, please speak to a Town staff for clarification.

Volunteer Work Habits:
-Reports to agreed upon shifts and is on time
-Satisfactory attendance record
-Follows policies and procedures as outlined in training
-Reports to shift wearing volunteer uniform and other appropriate attire as per dress code guidelines outlined in training

Customer Service:
-Responds to customers in a respectful manner at all times.

Interpersonal Relationships:
-Works co-operatively in a teach approach
-Respects knowledge, abilities and dignity of others
-Does not engage in any form of discrimination and/or harrassment

Communications:
-Keeps team lead and/or supervisor informed of activities and progress
-Encourages two-way communications

Technical Competence
-Follows direction given as per volunteer role
-Works in a safe manner at all times, as per health and safety training

This Confidentiality Agreement is made between the person listed above (hereinafter called "Volunteer") and the Corporation of the Town of Ajax (hereinafter called the "Employer.")

The volunteer may have access to personal and confidential information (i.e. participant attendance records, emergency contact information, etc.). This information is for use by the designated recreation program/event only and cannot be shared with the general public or used for any purpose other than the delivery of the Program. In receiving this personal information, the volunteer understands and promises to abide by the following terms and conditions:

1. The volunteer will not use the information for any purpose other than that required to conduct their role. An example of appropriate use would be taking daily participant attendance at a summer camp placement;

2. The volunteer will not give this personal information to anyone unless requried in the case of an emergency (i.e. Police, Fire, EMS, etc.). This includes discussing participant details or incidents that occurred on site with family or friends;

3. The volunteer will not use the information for the purpose of soliciting for other goods or services. An example of inappropriate use would be contacting participants to advertise a personal business;

4. The volunteer will not use the information to communicate with participants outside of the program (i.e. adding participants to Facebook, contacts in cell phone);

5. The volunteer will notify his/her supervisor immediately should they notice confidential information go missing and/or if the information has been accessed by someone other than a Town of Ajax employee.

Failure to follow the above terms and conditions may result in termination of volunteer placement.
